Deformity Templates
Templates help you launch faster by pre-building the questions and structure. You can start from a template, customize the fields, and publish when you are ready.
How to use a template
- Open the templates library
Visit the templates library to browse forms, quizzes, and calculators.
Use the category pills to jump between sections, or select Browse more to view every template in a category.
- Preview the template
Click any template to view the fields and try the live preview before you use it.
Toggle between AI and static mode to compare the experience before you decide.
- Use the template
Select Use this template to create a new Deformity in your workspace. If you are not logged in yet, you will be prompted to sign up or log in first.
